Abstract
Background: In recipients of liver transplantation (LT), spleen volume reflects the portal hemodynamic status. Optimization of graft weight and studying its effect on the outcomes on the splenic volume after the transplant is one of the techniques for portal inflow modulation. Aim: The study aims to assess the degree of splenic volume reduction after living donor liver transplantation in correlation to graft weight. Methods: The study analyzed 29 liver transplantation patients, involving comprehensive history taking, clinical examination, and laboratory data assessment. Spleen volume has been measured via a semi‑automatic region‑based technique with subsequent manual refinement when necessary to accurately delineate splenic borders and exclude adjacent structures. On axial CT images prior to and six months’ post-liver transplantation, with individual slice areas multiplied by their thickness and summed to obtain the total spleen volume.
Results: The mean Percentage of difference in splenic volume was 19.24 ± 6.33% with range between 7.69 and 31.11 %. The interclass agreement coefficient between Actual and radiological graft to recipient weight (GRWR) ratio is 0.865 (CI: 0.733 - 0.934) and this agreement illustrated high statistically significant value. There was mild statistically significant positive association among percentage of difference in splenic volume with graft weight and radiological graft to recipient weight ratio.
Conclusion: There is significant reduction in splenic volume after living donor liver transplantation (LDLT)The radiological assessment of graft-to-recipient weight ratio correlated significantly with splenic volume change after transplantation, so it could be used instead the actual graft to recipient weight ratio.
