Abstract

Background: End-stage renal disease (ESRD) occurs when the glomerular filtration rate falls below 15 mL/min, requiring renal replacement therapy. Despite improved survival, affected children experience significant physical, emotional, and social challenges. Consequently, health-related quality of life (HRQoL) is markedly reduced compared with healthy peers.

Aim: This study aimed to assess health-related quality of life (HRQOL) in pediatric patients with end-stage renal disease (ESRD) and to identify factors associate with the quality of life in pediatric patients with   end stage renal disease (ESRD).

Methods:  All 60 eligible children who were enrolled in the Nephrology Unit at Mansoura University Children's Hospital were included in the research. The Pediatric Quality of Life Inventory (PedsQL) was used in in-person interviews to gather data. The questionnaire assesses a variety of factors in many age groups (2–4, 5–7, 8–12, and 13–18 years), such as fatigue, concerns connected to renal illness, family and peer interaction, worry, physical appearance, and communication.

Results:  End-stage renal disease (ESRD) significantly impairs multiple domains of health-related quality of life (HRQoL) in children, particularly psychosocial and emotional dimensions. Adequate blood pressure control appears to be associated with improved HRQoL outcomes. A multidisciplinary approach addressing both medical and psychosocial aspects is essential to optimize patient well-being.

Conclusion:  Pediatric patients with end-stage renal disease experience impaired health-related quality of life across multiple domains, with variations according to age and clinical characteristics. Although most demographic and clinical factors showed limited associations with HRQoL, selected variables were associated with specific quality-of-life domains. These findings emphasize the need for comprehensive, multidisciplinary care that addresses both medical and psychosocial needs to improve HRQoL in pediatric patients with ESRD
